A lot of women experience this problem. There can be different causes: you might be wearing a badly constructed bra, or you have chosen the wrong style or size for your body type.
As far as the style is concerned, it is best to avoid deep balconnets. Extreme balconnets have a very low cut and therefore the shoulder straps are put at the farmost sides of the bra, near the wire. Choose a semi-balconnet or a full bra. The closer the shoulder straps are put together at the backside of the bra, the smaller the chance that they will slide off your shoulders.
The material of your bra is important too. If there is lycra inside, the back of your bra will fit better and give less opportunity to the shoulder straps to move.
Last but certainly not least: getting the right size is vital. If the bra is too big, the back panel will creep upwards and the shoulder straps will be loose and turned outwards. That automatically increases the probability of them falling off the shoulder.
